• >Ward rounds daily with consultant/registrar or by myself,

  > on call shifts where i clerking the patients including night shifts. on call ward cover shifts where i have managed all medicine wards in city hospital.

  >ensuring patients drug chart and prescriptions are correct
  >ensuring patient has discharge summary and TTO ready by the time patient is medically fit
  >ensuring other members of the team and colleagues are able to cope and offering help.
• > Reviewing CT scans and XRs or patients admitted with respiratory and cardiac abnormalities with consultant.
• > attending respiratory clinic with consultant and registrar
• > attending bronchoscopy with consultant

AMU Block : As part of my Job as SHO in respiratory medicine , i had to do 6 weeks AMU block every 3 months where i would be in acute medicine clerking patients being admitted in medicine and working as an SHO in Acute medicine, doing ward rounds in acute medicine. I also had to do night shifts where i will be holding the ward cover SHO bleep for medicine in the whole hospital covering 9 medicine wards including cardiology, hematology , respiratory and general medicine. I would go for emergency medical calls and do A to E exam and also assist with procedures like cannulation. During my AMU block i was fortunate enough to do procedures like Lumbar puncture apart from procedures like cannulation etc.

• Worked as JSD is elderly care in lyndon 5 ward. Daily ward rounds with consultants as well as supervising individual board rounds - seeing patients ,going through clerking and correlating the history from patients.
  Consultant led ward rounds where we observe the consultants seeing each patient - observing and learning case by case occasionally examining the patient under the supervision of consultant.
  
  Going through the necessary investigation (like scan , bloods , swab wound results etc. ) which again aid with the diagnosis.
  During individual ward rounds - Examining the patient on the next day where the consultant had seen the patient. to ensure patients treatment is not going in the wring direction.
  After the ward round, necessary and urgent investigations are sent , like bloods or inserting cannula if needed.
  Good communication with the nurses and other staff in the department ensuring that patients get stat doses and new drugs fluids etc that has been prescribed on time.
  
  Ensuring that patients who are medfit and being discharged the same day or next day has their discharge summaries prepared and TTO is done so that there wouldn’t be a delay in discharge. ensuring transport is organised.
  Proper communication with families and NOK - when patients especially during COVID times and working in elderly care who cannot properly speak to families, a proper family update is given and documented on a timely manner - this is done after ensuring patients have consented to sharing information with family members - where the current treatment plan is explained and when patients have no capacity in cases like dementia, delirium a collateral history is taken to know what their baseline cognitive function and mobility has been like for the past weeks / months.
  adequate communication with family,
  communicating adequately with other team members like Heart failure nurses, Tissue viability nurses, Nutrition team, Dietitians, Speech and language therapy team etc.
  
  AMU Block
  As RMO i used to take referrals from ED ensuring that the referrals are taken on time after discussing with ED doctor about the patient after making sure necessary investigations in ED are done and if the patients needs admission is established after coming to a conclusion of differential diagnosis. Some of the complex patients are discussed with registrar who might need immediate attention and ensured that these patients are seen on time and necessary investigations are sent off. If investigations like scans are needed on an urgent basis we speak to the scanning center and make sure that these patients are screened on a timely manner.
  During clerking major role is to ensure that patients are seen on time, proper history is taken and examined. If needed an urgent collateral from NOK or nursing/care home is taken so that patients care and mobility status can be well understood.

• my clinical attachment ( Shadowing ) in Elderly care department in Poole Hospital NHS trust Dorset for 6 weeks where i shadow the consultants and registrars while going for rounds and assessing new patients in the ward. I also tried to learn for the doctors here about NHS system and how the hospitals work and how computer log ins are used extensively in the system.
  1) exposed to acute unselected medical intake and triage in the UK
  2) Clerking the patients under assistance o consultant and SHO
  3) shadowed on call SHO in acute medicine and emergency department. assisted them in tasks such as clerking, ward rounds, TTO and operating computer.
  4) witnessed and learned to understand various native english accents with the ethnic mixture the region offers
  5) got accustomed to online patient data entry software
  6)learned to operate flowsheet , NEWS chart and imaging study results online.
  7) entered ward round notes under guidance of consultant, registrars and SHO
  8) attended ward rounds
  9) carefully listened to telephone conversations
  10) grabbed experience in communicating with patient in layman language
  11) attended teaching sessions from different consultants and SHOs on topics like pneumonia, osteoarthritis and how to read CT brain scans and CTPA scans.
  12) observed and was a part of daily handovers in different jobs
  13) understood the importance of guidelines in management and how different scoring systems and to be used

• i worked with Apollo tele health services in India as a junior doctor under project Jharkhand,India. Jharkhand in a rural state in north India, where there is a large population of people below poverty line in rural communities who don't have direct access to healthcare. they have to travel far away to have access to hospital services sometimes which is not practical for them. Hence Apollo hospitals came up with the initiate to start this project Jharkhand, where we set up a primary health clinic with a few nurses and staff to operate the tele video consultation to doctors from different parts of the country. Medicine were provided in the clinic itself and the consultation was almost free for most patients as they couldn't afford it. i got the opportunity to work with Apollo in this great initiate and treat patients who haven't even seen a doctor for a very long time. Most cases were General medicine, dermatology, trauma, gastroenterology, Paediatrics, Respiratory medicine. Through this platform i was able to realise how technology can be used to treat not just people in cities and towns but also people who have hardly any access to healthcare.

• i was working as a junior doctor in humaine health clinics looking after patients and evaluating lab reports of patients, doing the necessary investigations and referrals to appropriate departments.
  1) identifying and transferring patients whenever needed 2) stabilising patients by medications
  3) handling angry patients and relatives
  4) patient education
  5) treating patients for different medical conditions and follow up
  6) doing necessary investigations
  7)service improvement with weekly internal audits and implementing changes thereafter

• i was doing tele consultation in practo technologies which is an online tele consulting platform for doctors, here i was consulting patients online through audio and video calls, attending cases from all departments and guiding them on which doctor to see and what care is to be taken. Handling international patients in clearing their queries and doubts about various medical conditions, acting as an in house doctor in practo trying to improve the online medical consulting platform so healthcare reaches all borders, together with gaining experience in tele consult and how technology and medicine can benefit each other.

• As Medical doctor in Medlife int. Pvt. LTD for handling e-consultations(online consultations) where patients are consulted through phone calls, checking on their health status, discussions on their present health conditions and giving proper advice regarding betterment of their health. -handled few medical camps organised by the company including H1N1 vaccination camp, HPV vaccination camp and general doctor consultation camps in bangalore.

• I worked in PVS hospital kaloor, kochi as a junior doctor under consultant orthopaedician where i handled orthopaedic and trauma cases for acute and critical injuries in A&E including different types of fracture management, dislocation correction, wound sutures and wound debridement under local anaesthesia and general muscle and trauma pain managements. my work was supervised by my consultant.
  I also handled ICU Care for critical patients as a junior doctor discussing case scenarios with the senior doctors present in ICU and referred consultations from other departments.-Assisted senior consultant for several orthopaedic and trauma surgical procedures in the Operation Theatre.
  -Attended several CME with senior consultant, prepared and presented rare case management in Cochin Orthopaedic association.
  -Attended several camps organised by the hospital with a consultant led team in rural areas spreading knowledge about orthopaedic cases like osteoarthritis and its care n management. - part time casualty medical officer also in PVS memorial hospital.

• I did my mandatory rotatory internship in government general hospital, ernakulam. i opted to do my internship in Government general hospital , ernakulam, which is an NABH( National accreditation board for hospitals, india) accredited government hospital.
  My consultants taught me how a junior doctor should approach a case and how escalating to seniors is important in emergencies. i have gained confidence in history taking and i was able to fully grasp different procedures like cannulation, ABG sampling , urethral catheterisation, NG tube insertion, pleural tapping, suturing, dressing wounds, draining infected wounds, nasogastric tube insertion, otoscope, speculum, CPR, examination of - chest, abdomen, neurological, testicular, per vaginal, per rectal.
  The overall responsibilities in different departments ( medicine - surgery - gynaecology and obstetrics - paediatrics ) were about managing the in patient ward under senior supervision like maintaining patients medical records/charts also taking history , going for rounds daily with consultant , doing examinations and checking their vital signs, in addition to order the appropriate investigations , prescribing and administering medications to treat common health problems. Moreover managing patents in emergency department.
  -Casualty management including trauma care and emergency handling almost all cases from all departments specially cardiology and nephrology cases and management.
  -Have handled patients in several community healthcare camps serving rural communities and people below poverty line in kerala under a consultant led team.
  -Well trained and active member of basic life support team. General medicine ( 2 months ) : examine, collect , record, maintain patient information. additionally analyse records, reports, test results or examination information to diagnose the medical condition of the patient. i had the opportunity to learn both acute medicine and long term management of medical conditions.

General Surgery (2 months): Taking history, examining and order investigation for surgical emergencies like Appendicitis, Intestinal obstruction and inform seniors about the patients. Also, evaluation of the patient pre and post-operative and checking for any complications of the patients. Moreover, I have attended and assisted in different surgeries such as Appendectomy, Lipoma, Volvulus and Intussusception.

Gynaecology and Obstetrics (2 months): Examine, collect, record, maintain patient information such as taking a medical history, reports and examination. Also, provide care for women during prenatal, natal and post-natal periods. Additionally, attending cesarean section and normal vaginal deliveries.

Paediatrics (1 month): Assess children growth and development regularly, take a history, order investigations and perform physical examinations to reach a diagnosis in both paediatric ward and neonatal intensive care.

Community Medicine (2 months) : As an intern in rural community health centres where i under consultant supervision treated patients daily as out patient and in patient departments mostly diabetic, hypertensive follow ups, acute medical cases like viral infections etc , ensure the necessary treatment is provided under senior supervision, ensure the transfer of critically ill patients to tertiary centres , take classes in the rural community on topics like myocardial infarction, smoking etc, visit children's rural nursery to teach children about basic hygiene and ensure food and nutrition is adequately provided.
